Disability Services for Adults

Intensive Community Based Residential (Group Home) Services

AWARE's residential services for adults with developmental disabilities provide an array of supports to individuals whose challenging behavior, more limited skills, and/or chronic health conditions necessitate more intensive care, resources, services, rehabilitation, and/or guidance and supervision. The goals of these services are to enhance an individual's ability to benefit from services and activities, increase his or her independence, encourage full participation in community life, and prepare people to transition to less restrictive residential services. People residing in group homes are supplied with safe, comfortable housing, provided a well-balanced diet, daily living skills, assistance with medical concerns, group/individual therapy, and assisted in developing an individualized strength - based treatment plan. Program staff provide personal care and intervention for adults with mental health needs who cannot be served in a less restrictive environment. People living in group homes share the responsibilities of daily household routines. Adults are supported to learn skills related to self care, domestic living, and leisure/recreation.

Supported Employment Services

AWARE's Supported Employment services provide job placement for people in the community. These services include job development and job coaching, along with support to employers to ensure job completion. They provide opportunities for meaningful, paid work in integrated settings. These services also provide opportunities for the development of natural supports (supportive relationships with coworkers).

Supported Living Services

Our Supported Living Services provide an individualized array of supports, resources, and services that enable a person with disabilities to live in inclusive settings with experiences and routines that are typical of most people. AWARE's Supported Living services identify, provide, and coordinate necessary supports to maintain the person in the community and to create opportunities for him or her to participate in valued residential, work, medical and health, and social experiences.

Targeted Case Management Services

AWARE's Targeted Case Management provides assistance for adults with developmental disabilities in accessing and coordinating the services and supports that will allow each person to live at the highest quality of life. Case management services enhance an individual's independence and interdependence with other in the community, expand options for participation in - and contribution to - society, increase an individual's skills and abilities, and assist individuals to reside, duty nursing; social, leisure, and recreational supports; health, health maintenance, and health safety supports; and educational services.
