Affected by Mental Illness

Adult Mental Health Case Management

AWARE’s Adult Mental Health Case Management program helps clients and their families build upon their strengths, while cutting through the red tape that may have been preventing them excelling in the community. AWARE reaches out to adults and families affected by serious disabling mental illness, connecting them with the community agencies and resources to ensure that all the needed services are in place such as assessments, case planning and coordination, assistance in daily living, referrals, and advocacy. Every effort is made to help clients live successfully as full and productive members of the community.

Community Based Psychiatric Rehabilitation and Support

AWARE’s Community Based Psychiatric Rehabilitation and Support Services work with adults in all aspects of their daily and family life. Our qualified and caring staff help to support clients as they meet the challenges that may be preventing them from attaining their goals. AWARE helps clients in meeting their basic needs as related to living in the community, including housing, transportation, education, employment, social skill building, and any other areas that need attention as identified in a strength-based plan.

Any effort that helps clients meet challenges in these areas can be identified in the plan, and is supported by the program. Typically, these services are accessed through case management and/or in coordination with psychiatric care.

Intensive Community Based Rehabilitation

Consumers enter AWARE’s Intensive Community Based Rehabilitation program from either the Montana State Hospital at Warm Springs or the Montana Nursing Care Center in Lewistown. For many, an institution doesn’t adequately meet their needs, so they move into the AWARE Inc. ICBR. If the consumer decides they wish to remain for an indefinite amount of time, they may do so. Residents of ICBR are immersed in the community through organizations, activities, projects, or even school if they so desire. AWARE also assists if a consumer needs help finding a local job.

The program focuses on creating a normal home and integrating residents into the community. AWARE provides nursing services within the home to ensure that residents’ medical and psychiatric needs are met.

Individuals who are 65 or younger and who are affected by a seriously disabling mental illness diagnosis such as schizophrenia, bipolar disorder, personality disorder, or brain injury are eligible for this service. Consumers covered by Medicaid are eligible. Private pay arrangements are sometimes made.

Outpatient Clinical Services

AWARE promotes a "wraparound" philosophy so that services are strength-based, family focused, individualized, and comprehensive. Outpatient therapy is provided to youth, adults, and their families. Qualified professionals develop treatment plans suited to the individual or family, depending on their needs. Services are provided in a center (such as a clinic), at an AWARE office, or at an appropriate and confidential community site of the consumer’s choice.

At AWARE, the roles of Outpatient Clinical Services and Psychiatry are embedded within a broader therapeutic approach of unconditional care and individualized treatment, meaning that we will ensure that our clients are only receiving the treatments they need.

AWARE Psychiatric Services

As with our Outpatient Clinical Services, AWARE Psychiatric Services are embedded within a broader therapeutic approach of principles that emphasize strengths, family, community, and the individual. AWARE’s team of highly trained and experienced psychiatrists across Montana is available for assessments, ongoing treatment, medication management and support. AWARE is committed to access, and strives to provide psychiatry to even the most rural of Montana residents through telepsychiatry and video conferencing. To that end we have established an unparalleled network of mental health professionals.
