A.W.A.R.E. Inc. Youth Treatment Services -
Because A.W.A.R.E. promotes a "wrap-around" philosophy, services are strength-based, family-focused, individualized, and comprehensive. Depending on the level, strengths and needs of the individual, the various services are designed to provide different levels of support and supervision. The following is a brief description of the various services A.W.A.R.E. Inc. provides to individuals with developmental disabilities.

Youth Case Management: A.W.A.R.E. Youth Case Management services are provided to families, children and youth who need assistance to obtain and coordinate other community resources and services that will enhance the child's or youth's ability to remain with his or her family in the community and to participate fully in the community.
Case Management involves outreach to youth and families, while connecting them with numerous community agencies and resources to ensure that all needed services and supports are in place. By building an ongoing relationship, A.W.A.R.E. Youth Case Management accesses and coordinates the delivery of services and supports required for the individual young person and his or her family.
Services include assessment, case planning and coordination, assistance in daily living, referral and advocacy.
Youth Case Management is designed to empower parents to effectively advocate for their children with serious emotional disturbances by brokering services and providing support through the process of accessing those services. The case manager also helps families advocate for their children and family to use their own strengths and natural supports to accomplish their goals. The case manager assists with building and coordinating a strong team of people who will cooperate in developing a plan that builds on the child's strengths.
A.W.A.R.E., Inc. is licensed by the State of Montana as a mental health center and accredited by the Commission for the Accreditation of Rehabilitation Facilities (CARF).
Services are provided under Montanas Mental Health Services Plan (MHSP). Two categories of individuals are considered qualified for services under the MHSP - Medicaid eligible and non-Medicaid eligible - based on clinical and financial criteria.

| Family-based Support Services: These services offer a preservation program designed to keep or return youth to their famililes, providing support to families with severally emotional disturbed youth. Family based services serve any (1) child who is between 1-18, and (2) who meet the state criteria for severely emotional disturbed youth. The primary treatment goal of family based services is to provide the necessary amount of support to successfully treat that child in the community. Secondary goals focus on skill building in recreation, social, educational, and vocational arenas. Family Support Services offer families parenting education, skill building activities for children and youth, one-to-one therapeutic aides, counseling, children and youth groups, and respite. A strength-based approach is used with the parent(s) recognized as the leader of the treatment team. Services and action plans are individually designed by the family and their intervention team (a clinician, treatment specialist, treatment technician, and other community-based professionals). Services are provided in the family's home and in the community, as needed, seven days a week. In-home counseling support and 24-hour program coverage are available. Unconditional care and normalization philosophies are promoted. Staff is creative, flexible and available to meet the needs of the entire family.

Keeping Families Together
At A.W.A.R.E. Inc. we believe that the best place for a child is with their family. Our services are designed to allow severely emotionally disturbed children to remain at home. This means that FSS staff will assist you in exhausting every possible resource and support available to keep your child with you, in your home.

Family Centered Team
A.W.A.R.E. believes that family involvement is a crucial component of treatment. Professional staff will make every effort to involve the family in the treatment of the youth. We will help you build a strong team of people who will cooperate in developing a plan that builds on your childs strengths in order to meet theier special needs. This team will include the people in your community who play important roles in your childs care.

| You will be the leader of the team, and FSS staff will support you as you learn to effectively express your concerns and preference in serving your child. NO ONE knows your child as well as you, the parent. FSS staff will respect this and lead other members of the team to understand that, as well.

Comprehensive School and Community Treatment:
Our services make additional supports available for youth who are struggling with behavioral and emotional issues in their current school placement. Staff work closely with teachers and other school personnel in providing assessment, program planning, skill-building activities with the youth, one-to-one therapeutic support, and therapy based on the youth's individualized action plan. In addition, consultation, training, and support are offered to school personnel.

Psychiatric Services:
Psychiatric services are provided in the Bozeman area under contract by Dr. Jeffery Green. These services include psychiatric evaluation, medication services and management, consultation for families, case management and therapists. Dr. Green also participates in clinical reviews for partial hospitalization, residential treatment, therapeutic group home and family care and acute hospitalization placement. Psychiatric services can be accessed through the case management office.

Residential Therapeutic Services

Intensive Therapeutic Residential Care:
Intensive Therapeutic Group Home Services offer daily care and residential services for youth in a home-like environment. Four male or female youth are served in each home. Close supervision, needed structure, and individualized supports are provided by trained caregiving staff who implement each youth's strength-based treatment plan daily. In addition, youth participate in skill building activities, both individual sessions and therapeutic groups, and community-based activities. Family involvement is encouraged. Often youth transition from a therapeutic group home to therapeutic foster care, transitional living or family support services after approximately nine months.

Transitional Living Services: Our services provide supports to youth (ages 16-21) who need additional assistance learning the skills to live independently in an apartment. Skill building activities and supports, as outlined in the strength-based action plan, are provided for young adults as they transition to adult hood. Many of the youth served in this program have previously received care in foster homes or group homes. In addition, young adults who have dual diagnoses (cognitive delay and severe mental illness) receive supports through Transitional Living

Therapeutic Foster Care:
Therapeutic Foster Care offers daily care and residential services in a therapeutic environment for children and youth. The foster parents are professional caregivers who provide individualized supports for children or youth residing in their home based on a strength-based action plan.
In addition, a clinician, a treatment specialist, and a treatment technician provide support to the child or youth and the foster parents as the action plan is implemented.

A.W.A.R.E. believes the ideal situation for providing coordinated mental health servcies for young persons is to treat them in or as close to their home communities as possible. Because of a variety of factors that may not always be possible; however A.W.A.R.E. offers an alternative campus treatment program that combines education and a therapeutic group home setting.
