Case Management
Case Management helps adults, children and their families access services and support for both mental health and developmental disability needs. Together, our case managers and the people we serve assess needs, develop goals and monitor services. Individuals are then referred to their desired programs for assistance.
For example, an individual may be referred to health, social, leisure, recreational, cultural and/or educational programs.

AWARE’s targeted case management program serves more than 2,000 people statewide and is part of a program through the Montana Department for Public Health and Human Services.
